No, you can’t know every medication you are going to need, but if you are on a popular, patented maintenance drug such as <a target="_blank" href="../../web.php?q=lipitor&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Lipitor</a> or <a href="http://www.edrugsearch.com/web.php?q=cozaar&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Cozaar</a>, you’ll be pleased to hear that their patents are expiring in 2010, opening the field for more-affordable U.S. generic versions.</p>
<p>In the meantime, as you do your research, you will see that many are already available as generics, manufactured outside of the U.S. There is usually a considerable cost savings.</p>
<p>Here are all the details of some of the highest-profile drugs that have expiring patents as of the end of 2009, or during 2010 — and how and where to get the generic versions.</p>
<p><strong>ARICEPT (donepezil).</strong> Pfizer makes the acetylcholinesterase inhibitor <a target="_blank" href="http://www.edrugsearch.com/web.php?q=aricept&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Aricept</a> for the treatment of Alzheimer’s disease. <a href="../../web.php?q=aricept&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Aricept</a> is scheduled to lose patent protection in the U.S. in 2010, but <a href="http://www.edrugsearch.com/web.php?q=donepezil&amp;butSearch.x=17&amp;butSearch.y=12&amp;st=7">generic versions</a> are already available.</p>
<p><strong>COZAAR (losartan)</strong>. Merck’s <a href="http://www.edrugsearch.com/web.php?q=cozaar&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Cozaar</a> is an angiotensin II receptor antagonist, used to treat hypertension. <a href="http://www.edrugsearch.com/web.php?q=cozaar&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Cozaar</a>’s patent protection in the U.S. will expire in 2010. In the meantime, <a href="http://www.edrugsearch.com/web.php?q=losartan&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">generic losartan</a> is available <a href="http://www.edrugsearch.com/web.php?q=losartan&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">here</a>.</p>
<p><strong>LEVAQUIN (levofloxacin). </strong>Johnson &amp; Johnson’s antibiotic drug <a href="http://www.edrugsearch.com/web.php?q=levaquin&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Levaquin</a> will outlive its patent protection in 2010. However, until that time, you can purchase generic <a href="http://www.edrugsearch.com/web.php?q=levofloxacin&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">levofloxacin here</a>.</p>
<p><strong>LIPITOR (atorvastatin). </strong>Pfizer has the best-selling drug in the world in <a target="_blank" href="http://www.edrugsearch.com/web.php?q=lipitor&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Lipitor</a>, which treats high cholesterol. In the US, the basic patent for <a href="../../web.php?q=lipitor&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Lipitor</a> expires in March 2010, while the second patent covering the calcium salt of atorvastatin expires in June 2011. It is not yet clear exactly which date will see widespread availability of generic <a target="_blank" href="../../web.php?q=lipitor&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Lipitor</a> in the U.S., but it is coming. In the meantime, <a href="http://www.edrugsearch.com/web.php?q=atorvastatin&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">generic atorvastatin</a> is available <a href="http://www.edrugsearch.com/web.php?q=atorvastatin&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">here</a>.</p>
<p><strong>XENICAL (orlistat). </strong>Roche’s <a href="http://www.edrugsearch.com/web.php?q=xenical&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Xenical</a> is an obesity treatment that prevents the digestive system from absorbing fats. Its patent expires in December of this year.  (Note: In some countries, including the U.S., <a href="http://www.edrugsearch.com/web.php?q=orlistat&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">orlistat</a> is available over-the-counter as GlaxoSmithKline’s <a href="http://www.edrugsearch.com/web.php?q=alli&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Alli</a>.)</p>
<p><strong>VALTREX (valacyclovir). </strong>GlaxoSmithKline&#8217;s <a href="http://www.edrugsearch.com/web.php?q=valtrex&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Valtrex</a> is a treatment for herpes simplex and herpes zoster (shingles). <a href="http://www.edrugsearch.com/web.php?q=valacyclovir&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Valacyclovir</a> was recently approved for generic production, and so generic versions will be on the U.S. market by the end of 2009. Fortunately for people suffering from &#8221; eyelash hypotrichosis,&#8221; a brand-new treatment was approved by the FDA in December 2008. The product is called <a href="http://www.edrugsearch.com/web.php?q=latisse&amp;butSearch.x=41&amp;butSearch.y=17&amp;st=7">Latisse</a>, it has sultry-eyed Brooke Shields as a spokesmodel, and it is a 0.03% solution of bimatoprost marketed with an applicator brush.</p>
<p><a href="http://www.edrugsearch.com/web.php?q=latisse&amp;butSearch.x=41&amp;butSearch.y=17&amp;st=7">Latisse</a> is meant to be dabbed along the base of the upper lash line (the lower is too sensitive), causing hair there to grow in much more thickly and darkly than before. Its effects are not permanent, and if it is discontinued, the lashes will return to their original condition within weeks or months.</p>
<p>Technically, <a target="_blank" href="http://www.edrugsearch.com/web.php?q=Bimatoprost&amp;butSearch.x=45&amp;butSearch.y=17&amp;st=7">bimatoprost</a> is a prostaglandin analog that affects eye pressure. As a generic, and under the trade name <a href="http://www.edrugsearch.com/web.php?q=lumigan&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Lumigan</a>, the solution is used to control the progression of glaucoma and manage ocular hypertension. Although the main ingredient is the same, using <a href="http://www.edrugsearch.com/web.php?q=Bimatoprost&amp;butSearch.x=45&amp;butSearch.y=17&amp;st=7">bimatoprost</a> or <a href="http://www.edrugsearch.com/web.php?q=lumigan&amp;butSearch.x=0&amp;butSearch.y=0&amp;st=7">Lumigan</a>, which are serious ocular pressure meds, to lengthen or grow eyelashes is an unapproved, off-label use. (In fact, eyelash growth was a surprise side effect of these meds.) Some are tempted to swap these pharmaceuticals freely, but <a href="http://www.realself.com/question/latisse-bimatoprost">doctors warn that it can be harmful</a> to your health profile and insurance history to be have a history of ordering glaucoma drugs when you are merely trying to treat a cosmetic problem.</p>
<p>Commercials for <a target="_blank" href="http://www.edrugsearch.com/web.php?q=latisse&amp;butSearch.x=41&amp;butSearch.y=17&amp;st=7">Latisse</a> have been in heavy rotation on television and in glossy women’s magazines lately. If you aren’t paying close attention, you might think you are looking at a mascara ad, but mentions of the treatment being the “first and only” treatment approved by the FDA to treat hypotrichosis make it clear that this is not just make-up. <a href="http://www.mmm-online.com/ddmac-lashes-allergan-over-latisse-website/article/149134/">The glam may have gone a little too far, though</a>, says the FDA. <a target="_blank" href="http://www.msnbc.msn.com/id/32974056/ns/health-skin_and_beauty/">MSNBC reports</a>:</p>
<blockquote><p>Last week, Latisse made headlines when the FDA sent a warning letter to Allergan stating the promotional materials posted on the product’s Web site were “misleading because they omit and minimize risks associated with Latisse.” Among the risks, the FDA notes, is that the active ingredient can cause hair to grow in other places besides the lash area, cause inflammation of the cornea — and can make lighter-colored eyes turn brown.</p></blockquote>
<p>Meanwhile, <a target="_blank" href="http://industry.bnet.com/pharma/10004342/allergan-gets-warning-letter-on-latisse-over-too-small-risk-info/">an article at BNET</a> says the FDA also accused Allergan of downplaying infection risks.</p>
<p>We went directly to the <a target="_blank" href="http://www.latisse.com">Latisse website</a> to get their own description of the side effects. Here’s a summary: About 4% of users experience itchy and/or red eyes. Less common side effects include skin darkening, eye irritation, dryness of the eyes, and redness of the eyelids. It is very important not to use Latisse directly on the eye, or on the lower eyelid. It should only be applied to the UPPER eyelid with a sterile applicator. Latisse may cause darkening of the eyelid skin which may be reversible, and it may also cause increased brown pigmentation of the colored part of the eye which is likely to be permanent.</p>
<p>The information is there, but not particularly prominent. Changing the color of the eye is a huge side effect, and we’re curious how the public will react. Latisse is a very popular product, getting huge traffic on beauty boards as people compare notes on how to solve their lash problems without makeup. It will be interesting to see what develops as the FDA and Allergan come to an agreement on how to present side-effect information fairly.</p>

<p>There are six statins on the market: atorvastatin, fluvastatin, lovastatin, pravastatin, rosuvastatin, and simvastatin. They differ in their ability to reduce cholesterol, and they also differ in their rates of reducing heart attacks. Their costs are also quite different — and since most people take statins for a long time, the costs add up over the years. With all of these variables, <a target="_blank" href="http://www.ncbi.nlm.nih.gov/bookshelf/br.fcgi?book=ebm&amp;part=statins">choosing the right statin for you</a> can be complex.</p>
<p>All statins are capable of lowering LDL (“bad”) cholesterol and triglycerides, and raising HDL (“good”) cholesterol. The statins do differ in how effectively they can do this, and it is highly dose-dependent. <p>Indeed, the best-known statins are <a target="_blank" href="http://www.edrugsearch.com/compare-drug-prices-buy-online/Crestor">Crestor</a>, <a href="http://www.edrugsearch.com/compare-drug-prices-buy-online/Lipitor">Lipitor</a>, and <a href="http://www.edrugsearch.com/compare-drug-prices-buy-online/Zocor">Zocor</a> (quite probably because they have the greatest effect on cholesterol levels). The latter two are also endorsed by Consumer Reports. Taking evidence for effectiveness, safety, and cost into account, the publication rated both of these statins as “Consumer Reports Best Buy Drugs.” <a href="http://www.consumerreports.org/health/best-buy-drugs/statins.htm">They recommend</a>:</p>
<blockquote><p>• Generic simvastatin (20mg or 40 mg) — if you need 30% or greater LDL reduction and/or have heart disease or diabetes, or if you have had a heart attack or have acute coronary syndrome and your LDL level is not highly elevated.<br />
• Atorvastatin (Lipitor) (40mg or 80mg) — if you have had a heart attack or have acute coronary syndrome and your LDL is highly elevated; use for two years and then reconfirm need or switch to generic simvastatin.</p></blockquote>

For instance, they show that Lipitor (10-80 mg.) can reduce total cholesterol by 25-45%, while Zocor (5-80 mg.) can reduce the same numbers by 19-36%, and Crestor (5-40 mg.) can reduce it by 33-46%. As for lowering HDL, Lipitor can offer reduction of 5-9%, Zocor lessens HDL by 8-16%, and Crestor lowers these numbers by 8-14%. As you can see, choosing the proper statin has a lot to do with which numbers (Total Cholesterol, HDL, LDL, or triglycerides) you are trying to effect.</p>
<p>A final consideration is that last year there was reporting on an observational study done by Pfizer that suggested that there were certain benefits to using Lipitor over Crestor. However, one must keep in mind that Pfizer conducted the study, and they are the manufacturer of Lipitor, and they are defending this drug against Merck&#8217;s Zocor product, which is now available in a generic formula. Here is the information as presented by <a target="_blank" href="http://blogs.wsj.com/health/2008/01/14/lipitor-looks-better-than-zocor-in-retrospect/">The Wall Street Journal</a>:</p>
<blockquote><p>An analysis, published in the latest Clinical Therapeutics Journal, mined a large database of health-care records and found that patients taking Lipitor had a 12% lower risk of a cardiovascular event than those on simvastatin, the generic name for Zocor. The patients on Lipitor had a 15% lower risk of having a heart attack.</p>
<p>So-called observational studies like this one that look at data after the fact aren’t as powerful as prospective clinical trials. Jack Tu, a cardiologist who specializes in outcomes research at Canada’s Institute for Clinical Evaluative Sciences, says the latest Pfizer study didn’t take into account factors that could predispose a patient to heart problems, such as smoking and cholesterol levels. “Just on this alone, you wouldn’t recommend that everyone should switch onto Lipitor,” he says.</p>
<p>Still, Pfizer hopes that doctors will take notice. “We’ve done two rather large observational studies and patients have a lower risk of cardiovascular events on Lipitor [compared with] simvastatin,” says Susan Shiff, Pfizer’s team leader for cardiovascular outcomes. “Doctors need to factor this into discussions with patients.”</p></blockquote>
<p>You should definitely discuss with your physician which statin is right for you. In general, the best plan is to take the LOWEST dose of a statin that gets you to your target level for cholesterol. Overly large doses can be harmful to your liver and to your muscles. If you experience muscle aches and pains when taking a statin, contact your doctor immediately.</p>

<p>However, people suffering from <strong>atypical depression</strong> — characterized by hunger and sleepiness, rather than loss of appetite and insomnia — may have <a target="_blank" href="http://www.health.com/health/condition-article/0,,20188181,00.html">different needs in an antidepressant</a> and should discuss with their doctor options such as <a target="_blank" href="http://www.edrugsearch.com/web.php?q=bupropion">Bupropion</a> (familiarly known by its GlaxoSmithKline trade name, <a href="http://www.gsk.com/products/prescription_medicines/us/wellbutrin.htm">Wellbutrin</a>), which could help where popular SSRIs might fail.</p>
<p>That is because SSRIs — Selective Serotonin Reuptake Inhibitors — work on the neurotransmitter Serotonin, while <a target="_blank" href="http://www.edrugsearch.com/web.php?q=Wellbutrin">Wellbutrin</a> — an NDRI — is a Norepinephrine and Dopamine Reuptake Inhibitor. This change of focus in the affected neurotransmitter <a href="http://www.slate.com/id/2097682/">may make all the difference</a> for those individuals who suffer depression in an atypical manner.</p>
<p>Atypical depression differs from regular, “melancholic” depression primarily in energy and appetite. It is often characterized by “reversed vegetative symptoms” such as over-eating (regular depressives have little appetite) and/or over-sleeping (regular depression is usually characterized by sleeplessness).It is estimated that up to 40% of people suffering from depression have atypical symptoms.</p>
<p>Wellbutrin reportedly has a more enlivening affect than the SSRIs, so it is often considered for those who are sleeping abnormally long amounts. (For this reason, it is not advised for those suffering anxiety or agitation.) It is also one of the only antidepressants that is not associated with weight gain, and for this reason is also suitable for atypical depressives who may be overeating.</p>
<p>Even successful users of SSRIs have reason to consider Wellbutrin. SSRIs have widely-known sexual side effects that trouble some patients, even if the SSRI is curing their depression. This may make them turn to their physician for refinement of the prescription. Says <a target="_blank" href="http://www.raysahelian.com/bupropion.html">Ray Sahelian, M.D.</a>:</p>
<blockquote><p>Since bupropion raises dopamine levels, it actually can enhance sexuality rather than depress libido as do many SSRIs. Bupropion has been shown to have a favorable effect on sexual dysfunction. Several reports and small, short-term trials have examined bupropion as an adjunct or substitute for other antidepressants in sexually dysfunctional patients.  This means that Wellbutrin is often prescribed for sufferers of atypical depression, but it is also prescribed as an add-on for people using SSRIs who would like to try to combat the sexual side effects of SSRIs.</p></blockquote>
<p>In other words, Wellbutrin is often prescribed as an add-on to an SSRI for the express purpose of relieving some of the SSRI&#8217;s sexual side effects (usually loss of libido or functioning).</p>
<p>Depression is not the only condition that Bupropion has been approved to treat. In 1997, it was approved by the FDA as a smoking cessation aid under the name Zyban. And in 2006, Wellbutrin XL was approved as a treatment for Seasonal Affective Disorder. Because both Wellbutrin (prescribed for depression) and Zyban (prescribed for smoking cessation) are essentially Bupropion, patients should not take both at the same time; they would be doubling their dose.</p>
<p><em>Caution</em>: The most well-known side effect of Bupropion is that it lowers seizure thresholds. Doctors have worked around this with lower dosages (it is highly dose-dependent) and controlled-release formulations. Still, those with seizure disorders should avoid Bupropion. Other common side effects are nervousness/ agitation and insomnia. As with any antidepressant, people suffering from increased depressive symptoms or thoughts of suicide should contact their doctor immediately.</p>
<p>Some formulations of Bupropion are available as generics in the United States and Canada.</p>

<p>A Richmond, Virginia, doctor named Torino Jennings is accused of signing up to <a target="_blank" href="http://www.boston.com/news/local/massachusetts/articles/2009/05/29/va_doctor_accused_of_online_rx_violations_tax_fraud/">100,000 illegal prescriptions</a> for the muscle relaxant <a target="_blank" href="http://www.edrugsearch.com/web.php?q=soma">Soma</a>, as well as other drugs, over the past five years.  The prescriptions are illegal because he issued them on behalf of an <a href="http://www.noprescriptioneeded.com/">online pharmacy no prescription</a>,  without first examining the patients in person.</p>
<p>Many rogue online pharmacies aren’t technically “no prescription” pharmacies.  They have physicians, like Jennings, who write prescriptions for patients based on what are called “remote consultations.”  While the standards of remote-consultation pharmacies are generally higher than those of pharmacies that require no prescription, they do not meet the safety standards that you deserve — and should demand — as a patient.</p>
<p>Sometime back, the Washington Post ran a background check on remote-consultation physicians and reported &#8211;</p>
<blockquote><p>A Colorado doctor had a history of alcohol abuse. An Arkansas doctor was being treated for bipolar disorder and drug dependency. A Florida doctor had twice been cited for providing inadequate care to elderly patients, one of whom died. A Texas doctor was under investigation by the FBI for suspected Medicare fraud and later committed suicide. A California doctor was disciplined for operating under a fictitious name. A North Carolina doctor had held 22 jobs in five years</p></blockquote>
<p>When it comes to prescription requirements, there are three kinds of online pharmacies &#8212; two of which are both illegal and potentially dangerous:</p>
<p><strong>1. No Prescription Pharmacies.</strong> Some Web sites will sell you prescription drugs (including controlled substances) without a prescription, a practice that is illegal in the United States.  As <a target="_blank" href="http://www.pharmacychecker.com">PharmacyChecker.com</a> warns: </p>
<blockquote><p>Ordering from such sites is exceedingly risky to your health and could lead to unauthorized use of your credit card and/or identity theft. </p></blockquote>
<p><strong>2. Remote-Consultation Pharmacies.</strong> Some consumers wish to purchase drugs without having to visit a physician in person &#8212; either for convenience, to save money, or (in some cases) to obtain medications they might otherwise be unable to obtain.  Remote-consultation pharmacies prescribe medications based on the patient&#8217;s self-reported medical history.  Explains PharmacyChecker:</p>
<blockquote><p>U.S. states have different laws on what constitutes a valid doctor-patient medical consultation. As an advisory, the American Medical Association and the U.S. Food and Drug Administration have issued statements asserting that remote consultations are a substandard medical practice when there is no prior doctor-patient relationship, one established by an in-person physical exam. </p></blockquote>
<p><strong>3. Pharmacies Licensed by the U.S. or Canadian Government.</strong>  Licensed online pharmacies require you to provide an original prescription, obtained from an in-person meeting with your physician.  Licensed online pharmacies include major national chains, neighborhood pharmacies, and mail-order fulfillment centers in the United States and Canada.</p>

<p> <strong>1. Start with a heart-to-heart with your doctor</strong> &#8230; Tell him or her, honestly, how much you can afford to spend on medication &#8230; In the end, you may find you just don&#8217;t need some of the drugs you&#8217;re taking. </p>
<p><strong>2. Ask about generics.</strong> These are usually much, much cheaper than brand-name drugs, says Steven Findlay &#8230; The cholesterol-lowering drug <a href="http://www.edrugsearch.com/web.php?q=lipitor&#038;st=7">Lipitor</a>, he says, is a good example. <p><strong>5. Don&#8217;t &#8220;ask your doctor&#8221; like all those ads tell you to</strong>&#8230; <a href="http://www.edrugsearch.com/edsblog/study-big-pharmas-3-billion-in-annual-consumer-advertising-is-a-waste-of-money/">Advertised drugs</a> are expensive, and we often don&#8217;t need them.</p>
<p><strong>6. Be cautious about accepting free samples</strong>. Drug companies don&#8217;t give samples to doctors for every medication, just those that bring in the greatest profits&#8230;  </p>
<p><strong>7. Search the Web.</strong> You probably won&#8217;t find generics for less online, but you can save 10 to 30 percent on some brand-name medications if you buy them from <a href="http://www.edrugsearch.com/pharmacy-directory/">reputable drugstores that sell over the Internet</a>, Findlay says. &#8220;You will cut the brick-and-mortar &#8212; the overhead &#8212; costs,&#8221; he says. Shop around, though. Prices vary. </p>
<p><strong>8. Turn to Canada.</strong> Generics are cheaper here than there.
