The blood pressure lowering effects are largely attained within 1-2 weeks.

There are no trials of the Tekamlo (Aliskiren hemifumarate; amlodipine besylate) combination tablet demonstrating reductions in cardiovascular risk in patients with hypertension, but the amlodipine component has demonstrated such benefits.

The usual recommended starting dose of Tekamlo (Aliskiren hemifumarate; amlodipine besylate) is 150 mg/5 mg once daily as needed to control blood pressure. Titrate the dose to a maximum of 300 mg/10 mg once daily.

Tekamlo (Aliskiren hemifumarate; amlodipine besylate) is not recommended for use as initial therapy in patients with intravascular volume depletion .

Use Tekamlo (Aliskiren hemifumarate; amlodipine besylate) for patients not adequately controlled with aliskiren alone or amlodipine besylate (or another dihydropyridine calcium channel blocker) alone.

Switch a patient who experiences dose-limiting adverse reactions on either component alone to Tekamlo (Aliskiren hemifumarate; amlodipine besylate) containing a lower dose of that component in combination with the other to achieve similar blood pressure reductions.

Renal Impairment
Adjustment of the starting dose is not required in patients with mild to moderate renal impairment. Clinical experience with dosing Tekamlo (Aliskiren hemifumarate; amlodipine besylate) in patients with moderate renal impairment is limited. No data are available in patients with severe renal impairment .

Hepatic Impairment
No initial dosage adjustment is required for patients with mild or moderate liver insufficiency. Titrate slowly in patients with hepatic impairment .

Elderly Patients
Adjustment of the starting dose is not required for elderly patients.
